Unable to locate Report form.

Description: The report plug-in could not locate a form containing the reserved Report form fields.

Duplicate name for custom report. Do you want to overwrite the existing custom report?

Since the Log File Name field is not specified, the default file location and file name will be used for logging.

Select a report type.

If you close the report designer now, your changes will not be saved. Do you wish to continue?

You did not specify a form. In the Form field, select the form to use for the report.
